North East Community Support Services Caredove Utilization Report 2024

The North East Community Support Services (NECSS) Network has become a vital resource for connecting clients with the services they need in North Eastern Ontario. NECSS Network was designed to promote collaboration between agencies, reduce duplication of services, and ensure that all clients in the region have access to the support they need.

The NECSS Network has grown substantially since its launch in 2019. The Network now has 63 Community Support Service agencies listed with services, and 35 current active agencies. There are now 69 service categories to choose from and over 448 services listed on the NECSS Network.

The 2024 Caredove Usage Report for the NECSS Network highlights significant growth and increased utilization of the Caredove platform. The report shows a substantial rise in referrals, with a total of 7,787 referrals sent in 2024, marking a 2,050 referral increase from the previous year. The most referred service category was Hospital Transition to Home Support, accounting for 29% of all referrals. Despite the overall positive trend, certain service categories and organizations exhibited lower referral volumes, indicating areas for potential improvement. Moving forward, the focus will be on enhancing support for under-utilized services, promoting client self-referrals, and increasing referrals from family medical clinics to ensure comprehensive community support.

Job Description: First Link Intake Coordinator

Job Summary

The First Link Intake Coordinator is responsible for connecting newly diagnosed people living with dementia and their families to the Alzheimer Society so that they can get the help they need as soon as possible. The role of the First Link Intake Coordinator is to act as the first point of contact for clients who are referred into the Society, initiating the assessment process by collecting basic demographic information and booking appointments for intake assessments. The role also requires the First Link Intake Coordinator is to promote awareness and to strengthen community partnerships and referral systems by engaging with Primary Care Partners to facilitate direct referrals into the Alzheimer Society.

Essential Duties and Responsibilities

• Establish contacts within Primary Care and other community partners to help facilitate

direct referrals to the society

• Develop and maintain a system for triaging clients for intake assessment

• Make initial calls to clients to gather basic demographic information and set up a client profile for further intake by First link Care Navigator

• Book appointments for clients with a First link Care Navigator for further assessment

• Ensure that client files within the client database contain all required

client demographic information

• Complete client intake process including receiving fax and web-based referrals and reviewing relevant referral information

• Oversee internal and external referral processes

• Monitor referral sources; identify and address gaps

• Maintain and submit statistics as required

• Work in conjunction with other Society staff

• Perform other duties consistent with the job classification, as required

• Provides follow up letters to referral partners as per agency standards indicating the status of their referral

• Enter statistical & client data into electronic client database as per

organization standards

· Administer cognitive assessments using standardized tools

Hours of Work

The First Link Intake Coordinator generally completes all duties within normal business hours; however, flexibility in hours is required, with occasional evening and weekend work.

Job Qualifications

Education:

Minimum Bachelor degree in social work, gerontology or other related health care discipline. Registered health professional designation.

Experience

• Experience in office administration is considered an asset

• Experience in the health care and/or social service sectors is considered an asset

Other knowledge, skills, Abilities or Certifications:

• Excellent verbal and written communication skills in English

• Database experience an asset

• High standard of ethics and confidentiality

• Highly organized

• Knowledge of Alzheimer’s disease and other dementias

• Excellent communication skills (verbal and written)

• Exceptional interpersonal skills, including shared decision-making and facilitation

• Ability to prioritize workload and manage competing tasks

• Ability to take initiative and be resourceful

• Proficiency in technology (e.g.: Microsoft office and presentation software)

• Demonstrated ability to work independently and within a team

• Expertise and experience in cultural sensitivity and diversity

• Ability to speak French and other languages an asset

Travel Requirements:

Some travel in and around Algoma District to carry out the functions of this position, and to attend relevant meetings. Occasional travel outside of Algoma District may be required.

Valid driver’s license and access to a dependable vehicle are required.

Physical Demands:

• Performance of general office duties

• significant periods sitting at the computer, but with the opportunity to move away from the work station

• Sensory attention is required for the majority of the work day (looking at computer screen, reading documents, etc.)

Cyber Security North East Status Report

The Northern Cyber Security organization leads, Thunder Bay Regional Health Sciences Centre (North West) and Health Sciences North (North East) function as the Local Delivery Group (LDG) for their respective region. May’s monthly report for the North East has been provided by the LDGs to Ontario Health on the progress of deliverables.

The Cloud and Third-Party Vendor Management Workshop

On May 27, thirteen hospitals across the province participated in a workshop and tabletop exercise focused on securing cloud environments, managing vendor risks effectively and identifying tools and resources needed to assess third-party security. Members of the ten Local Delivery Groups, the Ontario Health Cyber Security Centre, the Ministry of Public and Business Service Delivery and Procurement’s Cyber Security Centre of Excellence and Rogers Cybersecure Catalyst were in attendance.

First Reading of the Connected Care for Canadians Act Completed

The House of Commons completed the first reading of Bill C-72, an Act respecting the interoperability of health information technology and to prohibit data blocking by health information technology vendors.

The Parliament of Canada’s website provides the current status, progress and details of Bill C-72.

The Government of Canada has also provided a news release regarding Bill C-72.

2023 Canadian Digital Health Survey Results

The Canada Health Infoway have concluded their 2023 Canadian Digital Health Survey and compiled the results. The survey gathered comprehensive data on the utilization of digital health services. It aimed to understand the factors that influence access and the patterns of use among patients.

We are very pleased to share that community-based health service providers (HSPs) across Ontario will now have access to free interRAI assessor and Integrated Assessment Record (IAR) training and support.

The Home & Community Support Innovation Centre will continue to support North East health service providers by providing regional training opportunities in the coming months. In the meantime, province-wide interRAI assessor and IAR training is available.

In a heartwarming display of community care, the Quilters of North Bay recently demonstrated their boundless generosity by donating over 80 beautifully crafted placemats to the Meals on Wheels program. Their thoughtful gesture is set to bring comfort and joy to North Bay VON’s MOW clients.

The intricate designs and meticulous craftsmanship of these placemats not only showcase the talent of the quilters but also serve as a tangible expression of compassion and support for those receiving meals through the program. Each placemat carries with it a message of warmth and connection, reminding clients that they are valued members of the community.

Seniors Community Grant Program Applications Now Open

Applications are now open for the Ontario Seniors Community Grant Program. The seniors grants support local, not-for-profit community groups and organizations to deliver programs and learning opportunities for Ontarians aged 55+.

Every year, the Ontario government provides hundreds of Seniors Community Grants ranging from $1,000 to $25,000 each. This funding allows community groups across the province to offer a wide variety of activities that help seniors to live independently, ensure their safety and security, connect them to their community, avoid isolation and help them achieve greater financial security and social connections.

Description: The Seniors Community Grants range from $1,000 to $25,000 and support community organizations in providing opportunities for social inclusion, volunteerism, and engagement for older adults, emphasizing safety.
Deadline : Applications must be submitted by Thursday, March 28, at 5:00 p.m. ET.
Eligibility : Grants are available for both incorporated organizations (up to $25,000) and individuals representing local unincorporated seniors’ groups (up to $10,000). Projects must involve older adults and align with the ministry’s priorities.
Programming : Projects can be delivered remotely, virtually, or in-person.
Timeline : Approved projects should begin by June 2024 and conclude by March 31, 2025.
Program Guidelines : Detailed guidelines are available in the “Ministry for Seniors and Accessibility, Seniors Community Grants 2024-2025 Program Guidelines.”

My Way Home Winter Preparedness Initiative

The Innovation Centre and Ontario Health North East are delighted to share that our Winter Preparedness Initiative was an overwhelming success! With the combined effort of 16 North East Community Support service organizations, we were able to support many individuals and families in the community.

Packages were delivered to 465 clients in the North East

The goal of the Winter Preparedness Initiative was to equip clients with essential items to ensure their safety and well-being throughout the winter season. Packages contained necessities such as warm blankets, mittens, and pantry items.

We are grateful for the support of all our partners who helped make this project possible.

We are excited to share that this initiative will continue throughout 2024 with plans for one initiative per season!

We look forward to continuing to work together to ensure the best care and support for our clients.

Charting the Course to Success: A Strategic Planning Course for Community Health Organizations

In the evolving landscape of Ontario’s health care system, strategic planning is crucial. But many community-based organizations struggle to lead an efficient strategic planning process that engages stakeholders and boards successfully.

OCSA’s LeaderShift division has partnered with Santis Health to launch a new online program called “Charting the Course to Success: A Strategic Planning Course for Community Health Organizations”. News Release: Ontario Recruiting More PSWs into LTC and Home Care

Ontario is investing over $300 million to recruit and incentivize personal support workers (PSWs) in long-term care and home care. As part of the Your Health plan, this initiative aims to enhance care for residents and expand home care services. Incentives include up to $25,400 for PSW students and recent graduates, retroactive to April 1, 2023. The government is also investing in training programs, like the Learn and Earn Accelerated Program for PSWs in Long-Term Care. The goal is to ensure long-term care residents receive four hours of direct care daily by 2025. This aligns with the government’s broader plan to improve healthcare accessibility and workforce growth. Stakeholders, including the Ontario Personal Support Workers Association and George Brown College, commend these investments, emphasizing the positive impact on addressing staffing shortages and enhancing care for seniors. OCSA Community ReConnect 2023 Conference

The Home & Community Support Innovation Centre team had the privilege of participating in the recent Ontario Community Support Association (OCSA) Conference held in Markham earlier this month. This vibrant event brought together key players from the Home and Community Care Support sector from across Ontario. We were honoured to listen to inspiring speakers, participate in enlightening Lightening Labs and breakout sessions and get a glimpse of what’s happening across the province in Home Care, all while networking with our peers and colleagues.

Deborah Simon, CEO of OCSA, was an eloquent host and facilitator. She brings a wealth of experience in the field of home and community care, along with a broad background in the healthcare sector, encompassing acute, long-term care, and rehabilitation sectors.

During the first day of the conference, we were honored to listen as Ron Tite, CEO of Church + State , gave an enlightening keynote speech focused on his guiding “Think, Do, Say” philosophy and how that sentiment relates to Home and Community Care.

One of the conference’s most dynamic features was the Lightening Lab Breakout Sessions. Our team actively participated in several of these innovative brainstorming sessions, which covered a wide range of topics:

HHR (Human Resources)
Volunteerism
OHT (Ontario Health Teams)
HCC Redesign (Home and Community Care Redesign):
Technology

These Lightening Labs sessions provided a platform for participants to identify what’s working, pinpoint obstacles, and pose questions phrased as “how might we…” to problem solve and brainstorm innovative solutions to current obstacles in Home and Community Care.

Dr. Chika Stacy Oriuwa is a Canadian physician and advocate who has been recognized for her work in promoting diversity and inclusion in the medical field. On day two of the conference she gave an exceptional keynote speech about her story, providing hope and reassurance for POC in Home and Community Support.

The Innovation Centre proudly presented a poster at the Poster Exhibition during the OCSA conference. Our poster provides a brief overview of the region we serve, our main outputs, and how they align with our mission and vision. Seen here is Quality Lead, Nic Labine, who happily discussed all that we do at the Innovation Centre and how our service model can be useful beyond NE Ontario.

To close out the conference, Pam Stoikopoulos, CEO of Big Eye Innovation presented an overview of all the Lightening Labs from day one, and spoke about the value of the Lightning Lab process.

The OCSA Conference 2023 was a remarkable gathering of passionate individuals and organizations committed to advancing home and community care in Ontario. The Innovation Centre team was honoured and delighted to attend, and we have brought back ideas, resources, and connections that will help us to continue with our mission to serve the Home and Community Care sector in North Eastern Ontario.

OceanMD / Caredove Integration

Send a Caredove eReferral directly from Ocean

The Innovation Centre has been fielding inquiries regarding an e-mail that has been sent out from the Change Management Team at Ontario Health in regard to the Ocean/Caredove integration. We have since met with this team to clarify what the expectation is from the North East CSS sector.

The Ocean/Caredove integration allows Ocean users to send Caredove referrals directly through the Ocean system. This integration simplifies the referral process for Primary Care and enhances coordination between our service providers.

Key Points to Note: There is no action on your part, and you will not notice any changes.

The Ocean and Caredove integration has already been implemented for some CSS providers in the North East and we will continue to work with the Digital Health team to ensure all Caredove users are able to receive referrals from Ocean users.
There is no need for any additional actions on your part, as all management and functionality aspects are handled in the backend, ensuring a smooth experience for users.
You will continue to receive Caredove referrals as you have been accustomed to.

Grant Opportunities – The Village Collective Impact Project

The Village Collective Impact Project is launching the final grant opportunity!

Description: The Village Project is looking to award up to two collaboration grants of $20,000 each, and two seed grants of $5,000 each. Organizations that support Seniors and Indigenous Seniors and Elders are eligible to become a partner. We are looking for organizations that want to pilot ideas that help remove barriers for Seniors specifically in the areas of transportation and reaching unreached Seniors in our region. These are small grants support small-scale initiatives or actions that can improve inclusion efforts for Seniors. Grant partner organizations support the project for the length of the project.

Grant Application Portal open for Applications – October 2, 2023

Application deadline is October 20, 2023

Successful grant recipients notified by November 1, 2023

All grant recipients to sign Transfer Payment Agreement (TPA) contracts by November 15, 2023

Transfer Funds – November 30 – Fourth and Final Round Funds Distributed to Grant Organizations

Once you are into the portal, you will also see the TPA (transfer payment agreement). Both the application and the TPA must be uploaded to the portal. If you are the successful candidate the TPA will need to be signed to release your funds, so please review and have your legal department or leadership team approve in advance. The money will be e-transferred by the end of November.

When you support the Village Project, you are agreeing to use the funds you receive to support seniors in one of the following ways:

Increase and promote social participation & inclusion for Seniors and Indigenous Seniors 60+, their caregivers and families
Find new or better ways of doing things to improve the experience of seniors and Indigenous seniors and their families and caregivers
Respond to emerging issues for the older adults you support in the community
Try out innovative approaches to creating and promoting age friendly or dementia friendly spaces

Over the last year, our core commitment revolved around strategic priorities that aimed to elevate collaborative service delivery within our community. Through meticulous planning and dedicated efforts, we embarked on a transformative path to enhance the quality and accessibility of healthcare services for all. This encompassed the development of innovative collaborative service models, fortifying our network’s foundation, and crafting essential tools that pave the way for the success of our services.

Aiming to Bring Guidance: Advising Ontario Health

At the heart of our strategy lies the ambition to provide guidance and options that enrich the planning work of Ontario Health North. With this purpose in mind, we embarked on a series of proactive initiatives that reflect our commitment to inclusivity, diversity, and equitable service. These include:

Developing a work plan to address equity, diversity, and inclusion for service staff and clients.
Formed a Diversity, Equity, and Inclusion Work Group, Inaugural meeting held.
Created a quality improvement process with indicators.
Established a Leadership Council and embedded quality improvement into its Terms of Reference

Through a virtual Fall Meeting, a series of impactful updates, and the distribution of informative materials, we’ve paved the way for local tables to flourish. We’ve championed the creation of standardized templates, fostering clear communication and alignment of priorities between Regional and Local Tables. Our resolute focus on local tables has strengthened relationships and empowered participation within our broader regional network.

Refining Excellence: Expanding and Refining Operating Guidelines

Prioritizing the evolution of our Standard Operating Guidelines remains of utmost importance. We’ve dedicated ourselves to refining these guidelines to create a seamless and efficient service delivery experience. Our engagement in developing, implementing, and evaluating Standard Operating Guidelines (SOGs) underscores our commitment to continuous improvement.

In Year 1, our unwavering dedication to collaborative service models, engagement with Ontario Health Teams, network strengthening, and tool creation has propelled us forward. Our journey is marked by initiatives that promote diversity and inclusion, establish a culture of quality improvement, and empower local voices. As we look back on our progress, and move into the next year’s strategic priorities, our determination remains persistent in furthering the vision of a Network where CSS is accessible to all, characterized by equity, and upheld to the utmost standards of quality.

Assisted Living Services Policy Update

Exciting News! We are pleased to announce the updated 2023 Assisted Living Services (ALS) Policy!

After collaborative efforts with Ontario Health and Home and Community Care Support Services, the legacy policies governing the ALS program have been reviewed and updated. The ALS Working Group endorsed action on recommendations from the Auditor General’s 2021 audit, with valuable input from ALS Health Service Providers across the province and various associations.

Key Updates in the July 13, 2023 ALS Policy:

No more requirement to reside in supportive housing for clients with ABI, physical disability, or HIV/AIDS.
Removal of service maximums in line with the Connecting Care Act, 2019.
No restriction for ALS clients to be on a Long-Term Care waitlist.
Emphasizing consent, equity, cultural care, client choice, and independence.
Clearly identified roles for discharges and transitions.
Mandatory staff training in health and safety, including Infection Prevention and Control.
Improved process for complaints and appeals.
Reporting requirements for better evaluation and future planning.

Future Updates and Implementation: The ALS Policy will be reviewed annually for the first three years post-implementation to accommodate health transformation and address recommendations from the Auditor General. Ontario Health will support ALS Health Service Providers during implementation, ensuring compliance without compromising client or care provider safety.

Can’t B. Leith Bert is Retired!

We bid a heartfelt farewell to Bert Leith, who retired from the Innovation Centre in May 2023 after many years of dedicated service. Bert’s illustrious career spanned the Home and Community Care sector, and he spent the last few years working as part of the team at the Innovation Centre. His unwavering commitment to innovation and passion for making a difference have left an indelible mark on the Home and Community Care Sector and on all of us at HCSIC.

Beyond his professional achievements, Bert’s love for hunting and fishing was well-known to all who had the pleasure of working with him. He effortlessly balanced his work responsibilities with his adventurous spirit, finding joy and inspiration in the great outdoors.

As Bert begins his well-deserved retirement we extend our deepest gratitude for his exceptional contributions, his infectious enthusiasm, and the positive impact he has made on our team. We wish him joy, relaxation, and countless memorable experiences in this new chapter of his life. Enjoy your retirement, Bert!

Accelerating the impact of Ontario Health Teams to advance integrated health care and improve patient experience across the province

The Ministry of Health is setting new direction for Ontario Health Teams (OHTs) to support their progress towards maturity as they work to connect care and improve patient experience in their local communities.

The new direction is outlined in the guidance document Ontario Health Teams – The Path Forward . The direction promotes greater standardization, informed by lessons learned through OHT implementation to date. Areas of focus include:

Clinical priorities – to
harness the potential of OHTs to improve health outcomes provincially
Governance – to strengthen and
standardize how decisions are made and ensure the right voices are part
of OHTs
Operational capacity – to make
sure every OHT can sustain its back-office functions
Communications – to cultivate
consistency on public communications led by OHTs and support
awareness of their role in delivering better connected care

Ontario Health Teams – The Path Forward will help OHTs to take the next steps forward on the path to maturity, with the
support of the ministry and Ontario Health.

Launch of the Personal Support Worker Return of Service Program

Launch of the Personal Support Worker Return of Service Program (PSW ROS) 2022-23

Ontario Health is pleased to announce the launch of the Personal Support Worker Return of Service (PSW ROS) for 2022-23. This program provides a $5,000 incentive to eligible Personal Support Workers in exchange for a six-month commitment in the Long-Term Care or Home and Community Care sectors.

All long-term care and home and community care employers must submit a new Employer Application Form to participate in the 2022-23 PSW ROS, even if they were approved in the previous year. Ontario Health will support approved employers to post opportunities on HFOJobs.ca and direct eligible PSWs to apply for these positions

Self-Directed Caregiver Respite Pilot Program Expansion

The Regional Respite Caregiver Working Group is pleased to announce the regional expansion of the Self-Directed Caregiver Respite Pilot Program. First trialled in November 2020 by The Friends, the Alzheimer Society of Sudbury-Manitoulin North Bay & Districts, Alzheimer Society of Sault Ste. Marie & Algoma District, and the Alzheimer Society of Timmins-Porcupine District, this pilot program served to provide caregivers with their own funding resources.

The pilot targeted participants who resided in rural communities and had limited or no access to traditional HSP respite services. These individuals were granted a monthly allocation that could pay for unconventional respite services, for example, having a caregiver’s neighbour spend time playing cards with the client.

Based on the success of the initial pilot, the program will be offered to eligible caregivers within the Northeast, starting on October 3rd, 2022. Eligible caregivers may receive up to $1200 per fiscal year to direct their own flexible respite services with a provider of their choosing (e.g., Friends, Family, private Health Service Partners).

Referrals for the program will be received by the Home and Community Support Innovation Centre through the North East Community Support Services Network . Following a vetting process, the referral will be forwarded to one of four Caregiver Respite providers based on the client’s location. These four organizations will be responsible for the eligibility determination, care coordination, administration, and facilitation of the pilot.

Full information on the pilot program will be available through the Innovation Portal, which will include Standard Operational Guidelines, Process Maps, Standard Works, and a brief training module.

Cyber Security Awareness Month

Dear Colleagues,

As you know, Ontario’s Cyber Security division is responsible for delivering a comprehensive cyber risk management program that enables the Ontario government to operate securely and meet its service delivery commitments. A key component of achieving safe and secure government operations is through cyber education and awareness, including campaigns during Cyber Security Awareness Month, an internationally recognized campaign held every October to highlight the importance of cyber security.

This year’s 11th annual campaign in the Ontario Public Service (OPS) is themed Cyber Security Awareness Month 2022: Managing Digital Risks .

Throughout the month, participants will increase their cyber skills through weekly interactive games and videos that present simulated cyber security threats, defences, and response actions. Participants will also level up their knowledge and skills on various topics such as cyber security hygiene, phishing, password protection, and malware attacks to become an “OPS Cyber Galaxy Defender”. By participating in Cyber Month, participants will help make the OPS more cyber secure as they learn what daily actions can be taken to manage digital threats.

The Cyber Security Awareness Month campaign is an essential component to raising cyber security awareness within the OPS and learning how we can best protect ourselves and our organization through strong cyber practices. As participants complete weekly tasks and challenges, they will gain points and virtual tokens that increase their chances of winning exciting recognition rewards.

Accurate listings and up-to-date Caredove profiles are key elements in receiving the right referrals. When referrers are able to view the best information about your programs and services they’re better able to refer the right clients, which means you can spend more of your time with the clients instead of dealing with inappropriate referrals.

Three key reminders:

Ensure your services are active and up to date.
Double check service details and eligibility criteria.
Use the Service Status Message
The Service Status field is intended to communicate important, short messages, that might frequently change regarding your service.
Review user permissions
Remove any users that have left the agency.
Verify user permissions and contact information.

Connecting To The Right Care Just Got Easier

North East Support has brought together more than 70 home and community care providers in the Northeastern Ontario region.

The website offers an interactive referral platform that allows users to find and connect with services near them or in any other region in Northeast Ontario. They can also search by service type, such as home care, recreation programs, meal programs, transportation or day programs for seniors with dementia.

Northeastsupport.ca makes it easy to find and connect with services that help individuals live independently in their own homes or in a community setting. If you have a need for home or community support services, this website will help you search and connect with agencies offering services in your area.

North East Support is made possible with the collaboration of 70+ Community Support Service agencies in North East Ontario, working together as one to continuously improve the client and family experience. The Caredove eReferral management platform is a key component of this collaborative effort.

Caredove Ocean Integration

North East Community Support Service Listings Are Now Available on the Ocean Healthmap!

The eHealth Centre of Excellence; Caredove Inc; Ontario Health; and the Home and Community Support Innovation Centre have partnered to enable health care practitioners in the North East to send patient referrals electronically (eReferrals) to community support services agencies.

The Ocean eReferral solution is now integrated with Caredove’s referral management platform. Clinicians can send referrals for many community support service programs with ease by searching for the program/service of interest on the Ocean Healthmap and choosing “ Send eReferral ”.

The sender initiates the eReferral from the Ocean eReferral Network and the referral information is received and reviewed by the community support services provider in the Caredove platform. The community support services provider will assess the referral and will contact the patient to book an appointment if necessary.

MOH-Digital Health Division Project

“Axe the Fax” Assessment Project

The Ministry of Health (MOH) has initiated an analysis of the current state and drivers of fax use in the province’s health sector to conduct early planning to identify options for the elimination of fax use in Ontario’s health system.

To deliver on this assessment, the MOH has engaged Deloitte to execute three key phases of work in the upcoming months:

Phase 1: Current State Assessment: Provide an assessment of the current fax use in Ontario’s health system landscape, including the drivers for use, top fax use cases, and opportunities and challenges for change
Phase 2: Options Analysis: Conduct a thorough qualitative and quantitative analysis of implementation options that would best enable the shift away from fax use in the provincial health landscape
Phase 3: Early Planning Roadmap: Provide a potential plan for the provincial rollout of implementation options for the top fax use cases
